Blaine Forman

Blaine Allen Forman “Peanuts,” 75, of Morgantown, passed away peacefully at his home on Monday, July 18, 2022, with his daughters by his side. He was born July 1, 1947, to Harold Forman and Sara (Rodgers) Smith, who both precede him in death.

Peanuts was known and loved by many. He adored his cat, Pretty, and his best friend, Lucy Morrison. He enjoyed his bartending days at Gene’s Beer Garden, where he was a giver of perfectly poured draft and sarcastic one-liners. He was a lifelong NASCAR fan.

Blaine is survived by his brother, Mike Forman, of Leesburg, Fla.; daughters, Tracy (Randy) Pavalok, of Grant Town, and Kerry (Joshua) Kidder, of Fairmont; grandsons, Dylan and Kaden Pavalok and Miles Kidder; a niece, Kristy (Terry) Kelley; a nephew, David Forman, and a great-niece and two great-nephews. He was also preceded in death by his sister-in-law, Patty Forman.

The family would like to thank Dr. Darrell Saunders and the staff and nurses at the Mon Health Cancer Center for their care during his chemotherapy treatments over the past three years and the team at Amedisys Hospice Care for their assistance during his final days. Hastings Funeral Home is in charge of cremation services and a private inurnment will follow at Beverly Hills Memorial Gardens. A celebration of life will be held at a later date.
